PLEASE USE the report form below to e-mail DATWA the details of equipment held, or to advise of changes to equipment already reported (ie, its location has changed, it is no longer held, there are more or less items of this piece of equipment, etc). Submissions to DATWA will usually be available for searching within 2 working days.

Submit one form for every different item of adaptive technology held (see the Technology page if you are unsure of what you have). Only one submission is required for multiple items of the same type and model.

Please be as specific as possible when entering equipment details. Include Make, Model and Version details wherever possible. If it is not clear which item of adaptive equipment you are reporting we will e-mail you to clarify exactly what it is.

NOTE THAT after submitting a completed form for one item, you can hit the Back button on your browser and change the fields that differ for the next item and then resubmit the Form again. It is not necessary to Reset the form and rekey all fields for each piece of equipment.
